End of Life Care: Hospice Care in the Nursing Home

Nursing Home

Hospice is a way of living with comfort, dignity, hope, and support during advanced illness. Hospice seeks to provide for the individual’s special needs for physical, emotional, and spiritual care at the end of life. Hospice enhances the quality of life left to an individual by insuring that it is as pain free and comfortable as possible.

Not everyone can be cared for at home in the advanced stages of illness, but everyone deserves special care at this time. Hospice Care in Westchester & Putnam works
closely with the nursing home staff to ensure enhanced services for the nursing home resident needing hospice services.

Through a contractual agreement between agencies, the nursing home staff receives training in hospice care and our team is oriented to the facility. Both parties work
together to provide individualized care to the patient and their family.

Hospice Services

Hospice Care in Westchester & Putnam can provide or coordinate hospice services within a hospital or nursing home, so that the patient can receive all the specialized
services and added care we offer. Each patient of Hospice receives services to meet their individual needs, which may include:

  • Hospice Nurses: Our compassionate hospice nurses coordinate the plan of care with the nursing home staff—working with the primary physician and unit nurses on symptom management and pain control. They also meet regularly with the patient and the family. 
  • Continuous Nursing Care: If it is necessary to stabilize the patient’s comfort level or manage a medical crisis, continuous nursing at bedside can be provided. 
  • On-Call Nurse: A nurse is always available to talk about symptom management and pain control, even after normal business hours. 
  • Hospice Home Health Aides: One on one care is available when needed to support the care provided by the nursing home staff. 
  • Hospice Social Workers: Help is available for patients and families facing the emotional adjustments of advanced illness. Counselors can help clarify options and assist with decision making. 
  • Hospice Pastoral Care Coordinator: A Chaplain is available for spiritual comfort and counseling. 
  • Hospice Volunteers: Our trained volunteers are there to provide a friendly ear or to run small errands or help write letters. 
  • Bereavement Counselors: Support and counseling are offered in individual and group settings for up to a year. 
  • Hospital Care: If hospitalization is required, the hospice team will coordinate services at affiliated hospitals and visit the patient regularly. 
  • Medication/Palliative Care: Hospice Care will provide all medications necessary to manage symptoms through an affiliated pharmacy. Specialized medical equipment and supplies are also made available.
