Telehealth is a way for patients at home to have a visit from a VNA nurse using a video telephone and diagnostic system. The program supports direct care visits from VNA professionals with monitoring of chronic conditions using a Home Health Monitoring System.
Telehealth can improve patient care; help avoid emergency room visits or hospital readmittance; and improve patient compliance with the prescribed health care regimens.
Telehealth uses a compact Home Health Monitoring System and an ordinary telephone line to connect patients with nurses in the VNA office by video. Using the system, the patient and the nurse can see and hear each other as the patient uses simple diagnostic tools attached to the system to report on vital signs, weight, pulse, oxygen—even the appearance of wounds. It enhances case management and early detection of medical problems. Telehealth offers many benefits:
