Specialty Programs: Mental Health Home Care

VNA of Hudson Valley has a history of helping individuals heal in the comfort and familiarity of their own homes. The Mental Health Home Care Program is part of that tradition. Since its inception in 1993, the program has taken a holistic approach to meeting complex mental health needs of clients, striving to help individuals achieve and maintain the highest possible level of wellness and independence.

Our multidisciplinary team works closely with the patient’s doctors to ensure quality and continuity of emotional, psychological, and medical care. The program offers home
based services which support the stabilization of patients through skilled intervention, education, evaluation, monitoring, and liaison with community resources. It also
serves the needs of family members by offering short term counseling, ongoing education, and other support services.

  • The program offers skilled mental health consultation and case management services which support the stabilization of patients through nursing, short term counseling, education, evaluation, monitoring, and liaison with community resources.

  • An experienced Psychiatric Nurse assesses the patient in his or her home, develops a treatment plan, monitors the response and works with our multidisciplinary team of nurses, physical therapists, occupational therapists, speech therapists, social workers, and home health aides. Care is provided under the super vision of the patient’s personal psychiatrist or primary physician.

  • Patients can be referred by physicians, psychiatrists, case managers, discharge planners, hospitals, clergy, friends or family.
